Destiny Take Over

its one of those days when we can easily grab a passing opportunity. A few hours surfing the web with a powerfull keyword, corresponding with certain people, downloading materials and uploading resumes..and loo you have a fantastic opportunity put by the Almighty in your hands.

such a temptation it is that it could make us partially blind, not physically but rather emotionally. So it lead me to a $150 inevitable option although a quite large sum for me, you can not say it immposible. the main problem lay in the payment method. All in all the matter we're settled, a bit late but just fine and if you think of it, its kind of luck actually.



so there i am in a beautiful sunday morning, nervous, because i am going to attend a TOEFL iBT test without much practice, if you compare with others. I took my regular bag and some pencils, off i go, its 09.00. a well planned morning.

Disaster could happen and strike you cold in the heart without notice. that was what happen to me when i open my bag and could not find my ID card, latter i found it inside my scaner at home. No ID no entrace and you can forget your money..but then i remember..i still have another ID, the one issued by my parent's home local civil office.

i explain the situation to the exam supervisor, and handed the second ID with my ice-felt arms, he examine it and stated it will be acceptable..ahh what a relieve, its 09.45, and disaster is still looming. Long before the noon break i realise that my keys are not where it suposse to be, with a red face i raise an arm and confer with the supervisor, he give a smile and say no i can't go downstairs myself but he will.

nothing happen after that, my keys safe in my pocket this time, just doing my best with speaking and writing section. My wife laugh at me when i tell her what happen earlier, it was a grand day and silly things just happen after all preparation, people say we can't escape destiny. so its destiny then when i receive notice from ETS saying that i have a magnificent 102 out of 120.

it occured to me that, in order to gain we must lose first, no pain no gain, and its seem to become a habit of me..for example i have to lose my birth certificate, the original one, its just vanished and the reward is 6 year of medical school, destiny take over. Destiny is such a tricky matter, we can plan and work but result and outcome lay in divine authority, it is the reason that we must pray, preserve our hope and fear, khauf and raja'...

Caca's Multiple Aphtae

It so happen than i have to take care of Caca in this holiday season, for the past week before the holiday season she began to lose her already small appetite. she barely eat at all through the day lucky is that milk time is still steady

suddenly at wednesday she wake up with a cry and completely iritable mood, i notice that the lower lip is a bit swollen, quickly her mother assesed and found at least four tiny aphtae. oh know.. i said to myself..she can't be inherited it from me ! ( FYI : i am easily develop aphtae w/o a minor trauma ) it would be very painfull to bear

later we found out that the upper lips is also involved a total of 6 aphtae with 2 "mother aphtae" in each lip. my poor baby.. she can't even talk, so no milk ,no food, for the rest of the day, we try to use gentian violet tough, at night she conceded a glass of water and slept out of tired crying all day

the next morning is much of the same, may be to gain comfort she insist (by sign lang) to be hold by nobody but me, i tell you its a back breaking job, to carry her with her fav pillow, around the house and the neighbourhood, lucky she sometime ask to have a walk inside her three wheeler, lucky too that she agree to be accompany by her granpa, meanwhile i lay a bit..

sundown brings her tantrum back, because she insist to go outside, i am started to think for help, its red in the calendar, already Isya to late to find a doctor in town, so i think of ol' Cipto pediatric emergency ward, thanks to my fellow telepatist Intan for the help there. all ini all is that night i spent near 300 grand so caca can fall in sleep..

Friday armed with kenalog and a powerful dose of CTM, i won the day, caca conceded a bottle of milk and bread, with decreasing iritability still unable to or afraid to talk

Saturday even better we saw her smile again, another bread and a lot of strawberry milk from Real good a.k.a susu bantal :) i said to myself, i can cope with high fever, diarhea, constipation, allergic symptoms but please not multiple aphtae again for such a young baby, it hurts me deeply, physically and emotionaly.

Air Asia a Better Low Cost Carier

Last week I flew to Medan with Air Asia, and again at the return trip to Jakarta. It is my first time with this airline and a good chance to compare them with other low cost carier. In my opinion, on their effort to reduce cost Air Asia has develop a certain policy like the ticket policy that I found to be a great inovation.

Air Asia does not issue a bisnis-like ticket like we used to see with other airline such as Adam Air, Loin Air and others. In fact their ticket is a simple print out paper with our name print on it without a predesignated seat. I think this policy save them lots of fund. Without a predesignated seat passenger will compete for their favorite corner and this could reduce passenger satisfaction on the other hand everybody will get a seat eventually.

There are many way to satisfy customer but certainly not with draging them to a different terminal by a bus. Air Asia passenger check in at terminal 1A but board the airplane at terminal 2 for foreign airline, instead of taxiing the plane which require consideable amount of expenxive avtur, they simply move the passenger with a less expensive bus. Sacrifying a bit comfort in exchange with efficiency, utterly briliant !

A passenger who never before have experience with Air Asia should feel less comfortable with the mentioned policy. I assure you their steward and stewardess still as good looking and pretty as other airline and perform their duty as good as it is. If a delay is unavoidable they still gave us meal, and most important they did not get my baggage lost.

To conclude this, all I can say is although Air Asia handle things differently, and ordinary paper for ticket and a bus solar for avtur, they still can satisfy their passenger. I can offer no complaint so I think even they are not the best still I assure you they are better than the others.

Anamnesis

When a patient enters a medical facility, seeking help. A medical doctor will promptly ask for their identity, this is a first step which gathers personal data, name, age, sex, and other relevant basic data. this is a moment to develop a doctor-patient relationship.

"what is the problem ?" or similar question is the second important step that will eventually lead to a diagnosis. this is a main symptom question, it is the symptom that prompt the patient to seek help to a medical facility, the complaint that bother him/her very much. the answer to the question above will guide the doctor's mind and produce a series of new questions that leads to several possible diagnosis, it is called a differential diagnosis.

those new questions must describe the history of the main symptom, when does its first felt ?, does it accompany by other complaint ?, describe its character. the questions here must obtain a typical symptom of a suspected disease. Next is obtaining history of previously acquired disease and other existing disease including present drug consumption.

history of allergic is always reviewed, depend to the patient, we could also need history of pregnancy, immunization, growth and development, etcetera

with anamnesis we can limit the possible diagnosis to several disease only, before we enter the physical examination stage. the doctor bears in his mind those possbile diagnosis, and continue his/her search by looking other sign and symptom. the doctor may need certain laboratory examination to confirm or eliminate suspected disease resulting in a working diagnosis.
